OLYMPIA — The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Chelan County
In January 2023 the Chiropractic Commission reinstated with conditions the chiropractor license of Christie Marie Collier (CH60186568). Collier agreed to the conditions, which place her license on probation, and require she participate in a substance use evaluation and monitoring program. Collier is not allowed to be a solo practitioner while in the program, and she must have her supervisor submit semi-annual performance evaluations.
Clallam County
In January 2023 the Hearing and Speech Board charged hearing aid specialist Thomas S. Gilliam (HA00002496) with unprofessional conduct for allegedly practicing after his license expired.
Clark County
In November 2022 the Unlicensed Practice Program notified Tang Xinwen of its intent to issue a cease-and-desist order. Xinwen allegedly gave a client a massage without having a license.
Douglas County
In January 2023 the Nursing Assistant Program placed on probation the medical assistant license of Summer Lee Barnhill (CM60892264). Barnhill misused controlled substances and must enroll in a substance recovery and monitoring program.
Island County
In January 2023 the Chiropractic Commission withdrew charges against chiropractor Rondle Jay Bennett (CH00003565).
King County
In December 2022 the Examining Board of Psychology ended conditions on the psychologist license of Bruce Arthur Olson (PY00000889).
In December 2022 the Examining Board of Psychology ended conditions on the psychologist license of Stephen K. Greenhouse (PY00003708).
Kitsap County
In January 2023 the Chiropractic Commission charged chiropractor Brandon Steven Slaugh (CH60960132) with unprofessional conduct. Slaugh allegedly had sexual contact with a patient. He also allegedly inappropriately massaged a second patient’s gluteal muscles, accessed her records, and contacted her.
In January 2023 the Nursing Assistant Program withdrew charges against the certified nursing assistant license of Nancy Shirley Ferara (NC60276652).
In November 2022 the Unlicensed Practice Program notified Kierra Brobst of its intention to issue a cease-and-desist order. She allegedly performed duties at the facility where she worked that fall within the scope of a registered or certified medical assistant, but she does not possess either of those credentials.
Pierce County
In January 2023 the secretary of health granted with conditions a substance use disorder professional trainee license to Adam Dewayne Hash (CO61245826). Hash’s credential will be on probation for at least four years and a professional will supervise his work, which will be evaluated quarterly in reports to the Program. Between 2018 and 2020, Hash was convicted of multiple felonies, including unlawful possession of a stolen vehicle, theft, and burglary.
In January 2023 the secretary of health conditionally granted an agency affiliated counselor license to Therese Louise Paxton (CG61349337). Paxton agreed to the conditions, which include at least two years’ probation, professional supervision, and quarterly performance evaluations. In 2018, Paxton was convicted of assault, criminal trespass, and making a false statement to a police officer. In 2019, she was convicted of attempted bail jumping.
In December 2022 the secretary of health denied the registered nursing assistant application of Kayla Marie Vance (NA61296249) because she did not get a substance use evaluation as part of her application. In 2021, Vance’s pharmacy assistant credential was suspended for diverting controlled substances.
In January 2023 the secretary of health ended conditions on the substance use disorder professional trainee license of Jeremy Aaron Johansen (CO60872001).
Skagit County
In January 2023 the Nursing Commission issued an order that placed the registered nurse license of Michael Justin Guzman (RN60802248) on probation for at least 12 months. During that period, Guzman must ensure that his supervisor submit performance reports every three months, may not accept employment in health care without prior Commission approval, may not work a shift within 12 hours of the previous shift, may not work more than one different shift within a seven day period, may not work more overtime than is required by any union or employment contract, may not work in a community-based care setting, may only be employed in a setting where indirect supervision is provided, and may not function as a supervisor, head nurse, or charge nurse. Additionally, Guzman must complete sixteen hours of coursework. Guzman had given a patient medication outside the time the physician had ordered it be administered, failed to chart information and medical administration, and failed to monitor and record vital signs.
In December 2022 the secretary of health denied the substance use disorder trainee license application of Wendy L. Smith (CO61326720) after she did not agree to practice with conditions. In 2018, Smith was convicted of possession of a stolen motor vehicle and theft of a motor vehicle. She was also convicted of criminal trespass earlier that year.
Out of State
Missouri: In January 2023 the Nursing Commission ended conditions on the registered nurse and advanced registered nurse practitioner licenses of Kathy Ann Galvez (RN60706531, AP60706532-NP).
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
